IRTS Transfer of Clients and Records Policy Upon Closure

  • Client Information Transfer: In the event of closure, client information will be transferred to their preferred agency or, if none is listed, to SpringPath Mental Health Services.
  • Notification of Closure: All affected clients will be notified in writing at least 25 days prior to closure, with information on accessing their records and arranging a transfer to another provider.
  • Record Access: Clients can access their records by contacting the company or submitting a written request.
  • Closed Case Records: SpringPath Mental Health Services will accept and maintain closed case records, providing follow-up services as needed.
  • Transition Meeting: The company will arrange a transition meeting with SpringPath Mental Health Services to facilitate client intake.
  • Alternative Arrangements: If SpringPath cannot accommodate all clients, the company will notify the DHS licensor and the county for further instructions.
  • No Guarantee of Availability: Neither the company nor SpringPath can guarantee bed availability at the time of transfer.
  • Annual Review: The policy will be reviewed and signed annually by a company owner.
